__ZEPHYR__ preprocessor macro is a way for a (cross-platform)
application to test if it's built for Zephyr. Currently it's defined
by Makefile, so if an app uses it's own build system, it won't be
available. So, define it in the standard header too to cover such
a case.

Replace the existing Apache 2.0 boilerplate header with an SPDX tag
throughout the zephyr code tree. This patch was generated via a
script run over the master branch.
Also updated doc/porting/application.rst that had a dependency on
line numbers in a literal include.
Manually updated subsys/logging/sys_log.c that had a malformed
header in the original file. Also cleanup several cases that already
had a SPDX tag and we either got a duplicate or missed updating.

This allows current code to build but using the definitions from
kernel.h instead of the original headers.

use zephyr.h as the main include in applications, no need to have nano/micro
includes in applications and samples.
Inclusion of the proper kernel headers is be handled in the zephyr.h based
on the configured and used kernel.

This application is intended for automated testing. It used binary
protocol for communication over UART and can be used for driving
Bluetooth stack by external tester.
Currently only single command registering (init) GAP service is
implemented.

This patch adds a very simple application for demonstrating the use of
the bt_init() API. Right now this is just tested to work with qemu and
the HCI UART driver with the help of the btproxy tool on the host OS
side. More information is available in the samples/bluetooth/README
file.

This patch adds a simple HCI UART (H4) driver that currently maps to
the second UART in the system. The main intention is to use this
together with qemu for accessing the Bluetooth controller available on
the host OS side.
The H4 HCI transport protocol is perhaps the simplest of the standard
HCI transports. It consists of a single byte in the beginning of each
packet which indicates the type of the packet: HCI event, HCI command,
ACL data, or SCO data (which we don't use at the moment).
